I'll give you what I usually put:
username: Chiara
reason: dailies
decks: Korean 1, Rare 3

the username is obvs, the reason is why you're requesting the card(s), and the decks is simply what decks ytou want to pull from. For more info, you can check out the Card Guide in the Cards Care Cebnter - it should be pinned.
