After moving to this server, i did not run a db upgrade cause the database server is a newer version, caused one of our plugin to send multiple commands at the same time and thus the error, i have disabled the plugin just to be safe and ran optimize + repair on all our database, hopefully this should not cause it again.

As for the late response, it was at the period i was always asleep, i am from asia.
