The mod is pretty good. It's standalone, you don't need any other game files. It's a Horror Survival game, you can build weapons and you need to manage your resources. It's no easy game and you probably need modern hardware to run it in highest resolution. There's also a Retro Edition - it doesn't look as good as the standard Director's Cut but it runs way smoother on older hardware.

With the latest update the developer added a New Game+ mode to the game. The enemies are placed differently in the levels (there are more enemies too) and there's an invincible enemy that hunts you through the whole game.
So if the normal game isn't nerve-racking enough...

the invincible enemy sounds like the annoying deadspace 2 monster
he can be killed, but later on in the game
modern hardware makes sense

I started playing New Game+ and I had fun for a while. But at some point it got annoying because the later game is already rather difficult. I never played New Game+ completely.

The difficulty without New Game+ is ok. It's no shooter like DOOM or Quake where you run around and kill enemies easily. Most weapons in Total Chaos are melee weapons. There are guns but there's not enough ammo to shoot your way through the whole game. Later in the game there's even an enemy that you can kill, but you can't shoot it.

To be honest, I can't remember if it's possible to adjust the difficulty of the Total Chaos mod (it's been a while) and I don't know if there are difficulty settings in the Total Chaos remake. I didn't play the demo yet.
