When your state or country has no volcanoes. That happened in Nebraska on July 31, 2022, when a region of Nebraska received an unusual EAS alert used for volcano warnings. Unsurprisingly, Nebraska has no volcanoes (it is known for being very flat after all), so this warning just confused the residents. In the end, it turned out that the volcano warning was actually supposed to be a wildfire warning, but someone had put the wrong code in by mistake. This was one of only two cases where this alert was activated, with the other time also being by mistake (though the other time was for Hawaii, which while also incorrect would be a lot more realistic than Nebraska).
