back in the days during the black plague, cities had walls that surrounded them to protect the city from attackers. they could lock the city up at any time to prevent people from leaving or coming in. (kind of like attack on titan)

so when attackers came to attack cities, they would get dead infected plague bodies and catapult them over the city walls to infect the people with the black plague.
