I got a prescription for a new medication that's newer, and therefore we needed to submit an request to my insurance company to ask for them to cover it and for some reason they wont cover it. So because of that my doctor sent an appeal, but he also prescribed me a similar med that they're going to cover that's twice a expensive for them. Now they get to pay twice as much instead of just covering the one that's less expensive

I don't really know but we have different instances for insurance and health care. We usually just send forms we get with the bills to them and they cover basically everything
They usually ask for a sticker from the health care as well. They also cover other things like psychologists for some reasons (not all)

Oh yeah, for us not everythings covered. It really depends on price and some other factors, they tend to want to push certain medications so they wont cover ones that aren't them to try to push people to use those ones. Its weird.

pre existing is fairly easy the issue is insurance is tied to the job and not the person.
Mostly because it was added as a perk for recruiting long ago.
So you just have to find a job with a large number of ee's.

Once a person is evaluated alone with out a pool of folks then ofc sick people are not getting insurance they are sick already.
If insurance is going to cover sick people then they gotta have more healthy people to balance that out.
Insurance is gambling writ large everyone wants to use someone else's money to pay for themselves.

The concepts of health CARE and Insurance are conflated in most peoples minds.
